29 CFR  1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  71 Dewey Street, Worcester, MA: On or about March 19, 2026, the employer did not ensure that employees using personal fall arrest systems had their lanyards attached to their harness at a point in the center of the wearer's back, while working at heights greater than 6 feet during a residential construction project involving shingle replacement.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  71 Dewey Street, Worcester, MA: On or about March 19, 2026, the employer had not prepared a written certification record of fall protection training which included the name or other identity of the person trained, the signature of the person trained, date(s) the training was completed and the signature of the person who conducted the training, for employees who were installing a residential roof and exposed to fall hazards.
