29 CFR  1910.178(l)(2): The employer did not ensure that each operator had successfully completed the training required by  paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a power industrial truck:  a) On or about 04/01/2026  in the production area, employer did not train some employees who operated forklift trucks, such as, but not limited to a Mitsubishi Forklift. 29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):  a)  On or about 04/01/2026, in the production area of  business located at 340 West Main Street Rochester NY;  the employer did not certify some employees operating forklift truck/power industrial vehicles were trained and evaluated.  Employees were operating powered Industrial vehicles to load pallets and boxes and were exposed to struck by hazards.  NO ABATEMENT CERTIFICATION REQUIRED

29 CFR  1904.41(a)(2):29 CFR 1904.41(a)(2) Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form.  a) On or about 04/01/2026, the employer failed during calendar year 2025, to electronically submit information from their OSHA Form 300A or equivalent by 03/02/2026. The establishment employed 140 employees and is classified during calendar year 2025.   NO ABATEMENT CERTIFICATION REQUIRED
