29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling  Maintenance shop: On or about March 25, 2026, and at times prior thereto, a relocatable power strip (SENTRY brand name) was fixed to a window frame by screws and was being used in lieu of fixed wiring to power a fan and other equipment. A red extension cord was daisy chined with another brown extension cord and was plugged into the relocatable power strip. The power strip was not being used in accordance with any instructions included in the listing or labeling.

29 CFR  1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  Maintenance Shop: On or about March 25, 2026, and at times prior thereto An electrical panel not marked for voltage was blocked by a small card box.

29 CFR 1910.1200(a)(2): Employer did not develop a written hazard communication program for the workplace.  Desert Rose Casino: On or about March 25, 2026, and at times prior thereto CSHO observed in plain view a handwritten marking on a spray bottle that was carried by the housekeeper. The CSHO asked the housekeeper what was inside the bottle, and she stated it contains disinfectant. The bottle did not have a label as per the new Hazard Communication Program requirements.

29 CFR  1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information specified under paragraphs (f)(6)(i) through (ii) of this section:  On or about March 25, 2026, and at times prior thereto CSHO observed in plain view a handwritten marking on a spray bottle that was carried by the housekeeper. The CSHO asked the housekeeper what was inside the bottle, and she stated it contains disinfectant. The bottle did not have a label as per the new Hazard Communication Program requirements.in addition, Three spray bottles were in the genitors closet and were not labeled accordingly to indicate their contents and the hazards associated with their contents. Employees were exposed to skin and eye irritation.
