At 3000 Shelby Industrial Drive, in Apopka, FL: On or about April 13, 2026, the employer failed to report a work-related in-patient hospitalization as a result of a work related incident, in that an employee was impacted by a moving vehicle while crossing the road, occurring on March 29, 2026, to OSHA within 24 hours after learning about it.
