29 CFR  1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective.  An extension cord used to supply power to an outdoor industrial fan was missing the grounding (third) prong, thereby interrupting the equipment grounding path and exposing employees to electrical shock and electrocution hazards.
