29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) On or about April  23, 2026, the employer did not ensure employees engaged in preparing a roof for shingle installation with a 10/12 pitch at a residential home located at 19339 Riverview Ave., Rocky River, OH 20-25 feet above the ground were protected by guardrail systems, safety net system, or personal fall arrest system.  b) On or about April  23, 2026, the employer did not ensure employees engaged in preparing a roof for shingle installation with a 3/12 pitch at a residential home located at 19339 Riverview Ave., Rocky River, OH 20-25 feet above the ground were protected by guardrail systems, safety net system, personal fall arrest system, or safety monitoring.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about April 23, 2026, at the location of a worksite located at 19339 Riverview Ave., Rocky River, OH: employees were exposed to a fall hazard of approximately 22 feet while performing roofing activities on a residential home . Employees were using an extension ladder to access the roof, and the employer did not ensure they extended at least 3 feet above the upper landing surface.
