29 CFR  1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates and mud sills or other adequate firm foundation:   (a) Residence: On or about 4/14/2026, and at times prior, the employer exposed employees to fall hazards, in that the ladder?jack scaffold was erected using two portable Werner ladders that were not placed on an adequate, firm foundation or level surface while an employee was performing siding repair and installation.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.     (a) Residence: On or about 4/14/2026, and at times prior, the employer exposed employees to fall hazards of more than 9 feet from ground level, in that a safe access was not provided while an employee was performing siding repair  and installation.

29 CFR  1926.452(k)(4): Ladders used to support ladder jacks shall be placed, fastened, or equipped with devices to prevent slipping.   (a) Residence:  On or about 4/14/26 and times prior, the employer exposed employees to fall hazards of more than 9 feet to ground level in that a ladder jack scaffold erected with two portable Werner ladders did not have the base of the ladders placed at adequate angles to prevent slipping while an employee was performing siding activities.

29 CFR  1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.   (a) Residence:  On or about 4/14/26 and times prior, the employer exposed employees to fall hazards of more than 9 feet above ground level in that a ladder jack scaffold was not erected under the direction of a competent person who could identify hazards such as inadequate firm foundation, no safe access and incorrect ladder placement while an employee was using the ladder jack scaffold to perform siding activities.
