29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a.  On or about May 4, 2026, at the above addressed jobsite, employees performing residential roofing activities, nine (9) feet eight (8) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.

On or about May 4, 2026, at the above addressed jobsite, employees were using a portable extension ladder to egress the roof while carrying objects that could cause the employees to lose balance, thereby exposing the employees to fall hazards.

On or about May 4, 2026, at the above addressed jobsite, the employees had been provided fall protection training; however, the employer failed to verify the training by preparing a written certification of training record.
