An employee was lowering the frame of the bread rounder back into place on the conveyor and pinched their right thumb resulting in amputation of the thumb.

An employee was cutting top sirloin on a meat saw and amputated the right index finger.

An employee was unhooking a trailer from the truck and was pulling the 5th wheel pin to drop the trailer. The trailer was on a slight incline and when he pulled the pin, the trailer slid and dropped catching his right hand and causing lacerations.

An employee was sprayed with hot product causing third degree burns to his neck, upper torso and legs. He also sustained a dislocated right shoulder in the incident. The employee was admitted to the burn unit for treatment.

On December 30, 2016, an employee was burned on the face, arm and left hand while cleaning a jiffy mixer in a solvent tank when the solvent vapor ignited.

An employee was adjusting the cable on the electric hoist when his left index finger got caught up in the hoist cable and the fingertip was amputated.

An employee was walking into work from the parking lot, slipped on black ice and fell fracturing the pelvis and a wrist.

On December 30, 2016, at 5:45 am an employee slipped on black ice in the back parking lot and fell while using a flashlight on his way to the jockey truck to move a loaded trailer. The employee fractured the lower left leg that required surgery and hospitalization.

An employee was performing sanitation duties on a machine. The employee removed piping to clean a valve inside. The air source to the machine was not locked out. While cleaning the valve, it actuated and caught her finger. She was hospitalized and the finger may be amputated.

The employee was making parts at the Accupress machine and stacking them on a skid that was on a cart. The employee went to adjust the materials when the skid and parts fell on him, causing him to fall backwards with the material landing on his stomach and legs and his head striking machinery behind him. The employee was hospitalized for a fractured left kneecap and leg, a fractured right ankle and multiple lacerations to his legs, stomach and head.

An employee was moving a ladder and caught their right index finger in the hinge resulting in a fingertip amputation.

An employee was grinding parts on a pedestal belt grinder and suffered a left thumb amputation.

An employee was operating a bus when he crashed into an 8-foot fence, suffering lacerations and a fractured left shoulder.

An employee was using a hand grinder when he saw something wrong with the guard. When he attempted to adjust the guard, the grinder caught his gloved hand and amputated his left index finger. The grinder was still in operation at the time of the incident.

An employee was checking a cyclone machine when he opened the spy door and fire shot out, burning his forehead, cheek, and chin.

An employee's gloved hand was caught between the moving rollers while removing strings from the powered conveyor resulting in the amputation of his right finger above the first knuckle.

An employee was using the scanner, tripped over the scanner cord and fell fracturing a hip.

An employee was operating an LPM machine to seal small mustard packages. A mustard package jammed up the LPN machine and when the employee tried to pull the mustard package out, part of her left index finger was removed. The machine was guarded at the time.

An employee was unloading a truck using an electric pallet jack. As he was exiting the truck, the dock plate malfunctioned and pinned the employee's foot against the electric pallet jack causing lacerations and fractures to several bones in his right foot.

On December 29, 2016, at approximately 11:30 a.m., a crew was moving skids out of the bottom of a large excavation. The skids had been placed in an excavator's hoe bucket on the ground. An employee was unloading the skids from the bucket and handing them to the injured employee, who was positioned next to the track hoe bucket and a super sack of sandbags. At this time, another employee walked by the hoe operator and asked him to take over the track hoe. As he twisted around to speak, his safety vest caught the operating lever and caused the bucket of the track hoe to move and pinch the injured employee's left ankle between the hoe bucket and super sand bag. His lower left leg was fractured just above the ankle.

An employee was blending solvents (dichloroacetic acid and toluene) in a 100-liter glass vessel when it cracked, splashing solvent on the employee's left shoulder, chest, and back.
