On December 31, 2024, an employee was monitoring a rotary valve machine for the discharge of materials when four fingers on his right hand were amputated at the knuckle joint.

An employee parked their truck and was lowering the lift gate to unload and service an account at a convenience store. A vehicle pulled out from a parking spot and pinned the employee against the lift gate. The employee sustained a leg amputation.

An employee was unjamming a packaging machine when they sustained a partial thumb amputation (right thumb at the knuckle) when the cutter engaged. Lock out/tag out procedures were not used.

An employee was servicing a manlift when their little finger became caught in the machinery and the finger was amputated.

On December 30, 2024, at approximately 8:00 PM, an employee was troubleshooting a packaging machine that was jammed with plastic. When the employee went to pull the plastic out from the vacuum seal section, the machine actuated and his right middle and ring fingers were caught in the machine. The employee sustained partial amputations to both fingers above the first knuckles. The machine was energized and not locked out/tagged out at the time.

An employee was cutting a piece of meat using a band saw when he went to retrieve a piece of meat and sustained amputations of his left middle and ring fingers.

On December 30, 2024, at 8:15 a.m., a welder was lifting a 500-pound load of metal (a quad) using a crane. The load became unstable and their left ring finger was caught between the load and the crane hook, resulting in a fingertip amputation.

An employee was cutting a femur bone from an animal carcass when the band saw amputated part of his left ring finger.

On December 29, 2024, at 1:30 PM, an employee was using a safety knife to move an object from the conveyor when it became stuck in a pinch point between two rolls of an extrusion machine. The employee's right hand was pulled into the machine, resulting in amputation of the hand.

An employee was accessing a rotary airlock to clean it out when their fingers were caught by the star valve as it was rotating. The employee sustained amputations of their left middle and ring fingers.

An employee was cleaning and re-webbing a printing press. The employee's right index finger was pinched between the shaft of the printing press and the carriage bearing holder as they came together. The fingertip was amputated.

An employee was cutting meat with a meat saw when his hand slipped and the blade amputated his right thumb tip.

An employee was chopping bacon using a chef's knife and amputated their left thumb tip.

A crew was working to untangle a spool of 1/2-inch wire cable. One employee's left thumb became entangled in the cable, causing an amputation/avulsion. The employee was hospitalized.

An employee was clearing metal debris from a horizontal band saw after cutting a metal beam when their right glove contacted the blade, resulting in amputation of the right little finger and injuries to two other fingers.

An employee was cleaning an anilox roller on a printing press when his rag became caught in the rollers. The employee let go of the rag and took a step back, slipped on ink, and fell into the railing bar in front of the anilox roller. The roller bounced up and the employee's hand became caught underneath it as he fell. The employee sustained sustained amputations to the nail beds of two fingers and required stitches to his hand.

An employee was cleaning the nip roller and setting up/operating a laminating machine when their left hand was caught in the machine and crushed. The employee sustained the amputation of their middle, ring, and little fingers. The machine was not locked out/tagged out at the time.

An employee was using a hydraulic lift to get a steel box full of coins into a truck. The employee noticed the box was going to get caught on the bumper. He went to move the box and the hydraulic lift caught his thumb, resulting in partial amputation.

An employee was helping a customer when their finger became caught in a door jamb, resulting in a partial amputation.

An employee was packaging brick. A brick broke from the bundle and became lodged between the conveyor rollers. The employee was removing the brick when the bundle of brick shifted forward, catching his left index finger between the bundle and the conveyor roller and resulting in a partial amputation to the fingertip.

An employee was checking an inline magnet. The magnet was placed on a cart next to another magnet and the magnets moved toward each other. The employee's right ring fingertip was caught and amputated.

On December 24, 2024, an employee was moving a piece of cargo (wooden crate weighing approximately 300 lbs.) with another employee when the cargo slipped. His left middle finger was caught between the cargo and the belt loader, resulting in the amputation of the finger.

An employee was opening a roll-up door when his glove was caught in the door's rope, resulting in a partial amputation to his right index fingertip.

An employee was cutting tubing into " segments on an automatic band saw. He went to brush away scrap material and his left hand got caught in the blade. The employee's little finger was partially amputated, his ring and middle fingers were lacerated, and he required surgery. A guard was in place at the time.

An employee was applying a hoist hook to the I-bolt on a die plate when their right ring fingertip became pinched between the hook swivel and the bolt, resulting in an amputation/avulsion.
