An employee was standing on the third step of a 6-foot ladder, hanging a flood light on a house. He fell from the ladder to the ground, resulting in broken ribs.

On September 30, 2025, at 1:45 a.m., an employee was operating a forklift to load materials on an outbound dock. His left foot became caught between the forklift and a pole, resulting in a broken big toe.

An employee was working in a manlift, using a hydraulic torque wrench to tighten bolts on a frac stack. The wrench activated and caught his left middle finger against a crown nut, causing an amputation at the last knuckle.

An employee was burned by an arc flash that occurred in an electrical room.

He also sustained fractures to a hand and finger.

During a routine inspection, an employee placed their hand on a pipe to check a weld. The pipe clamp activated, closed on the pipe, and trapped the employee's right middle finger, resulting in a partial amputation and fracture to the fingertip as well as a laceration to the ring finger.

An employee was backing up a forklift when the frame of the forklift clipped the wall. The employee's right little finger was pinched between the wall and the forklift, resulting in amputation.

An employee was assisting a client when the client fell on top of the employee. The employee's hip was fractured.

An employee had been removing phosphorus pentasulfide while on a scaffold. He was using a non-sparking tool to dislodge the phosphoric material. The employee noticed a flash of flame. He descended the scaffold, but the flash ignited residual phosphorus pentasulfide that was on the outside of the employee's personal protective equipment (PPE). He sustained thermal burns to his right hand and right calf.

An employee was crossing over a cattle gate when the gate broke. The employee slipped and fell, landing on his ankle. The employee was hospitalized with a fractured ankle and required a procedure.

An employee was using a forklift to unload a trailer. The trailer moved away from the dock, causing the forklift to fall out and come to rest against the trailer and the dock plate. The employee was hospitalized with fractured lumbar and sacral vertebrae.

An employee was rigging up a location. He was using an Allen wrench to free up rotation and start the drawworks engine manually. The engine rotated and his left little finger became caught between the Allen wrench and the engine. The employee sustained an amputation to his little finger and fractures to his wrist.

An employee was working from a scaffold when the scaffold fell over onto the floor. The employee sustained fractures to their lower back and pelvis.

An employee was picking up scaffold frames when scaffold frames that were leaning against a concrete masonry unit (CMU) wall, fell onto the employee's back. The employee required surgery for a blood clot in their neck.

An employee had been having lunch in her car in the parking lot. When she exited her car, her left knee gave out and she fell on the ground. The employee was hospitalized with a fractured left femur and required surgery.

An employee was working from the elevated bucket of an excavator when they fell between two tanks and landed on the ground. The employee sustained a fractured leg.

An employee was hanging a small sign above the coffee bar in a gas station. He fell off a scaffold and landed on the floor. The employee sustained fractured ribs.

Employees were loading a 295-pound transformer onto a pickup truck using a forklift. The injured employee was acting as the spotter. The transformer hit the tailgate of the pickup truck and the spotter's right hand was struck by the transformer, resulting in amputation of the ring fingertip.

An employee was using a table saw to cut strips of plywood. The employee's left hand came into contact with the sawblade, which amputated the index and middle fingers as well as lacerating the hand. The employee was hospitalized, requiring surgery.
