An employee was removing boxes of filters from a skid when he tripped on a manual pallet jack. The employee fell and struck his back on the base of the pallet jack, resulting in hospitalization with fractured ribs.

An employee was walking through a parking garage. She tripped over a curb and fell forward onto her right knee, then her left knee, both hands, and her face. She suffered an injury to the left knee that required surgery, as well as scrapes on the chin and both palms and soreness in the left wrist.

An employee was using a hose to clear storm drains after a sewer backup. The employee fell backward and his head struck the curb, resulting in an Injury on the right side of his head, generalized bruising, and pain in his left ankle, right hip, left shoulder, and neck. The employee was hospitalized.

An employee was working on an exhaust fan on the roof when he tripped on a wire and fell to the ground. The employee sustained scrapes on his elbow, and a fractured right knee with a torn ACL. The employee was hospitalized.

An employee was replacing the back bracket on a deep fryer when her screwdriver slipped, causing her to fall forward into hot oil. She suffered burns on both arms up to the elbows (including the hands and fingers), as well as on the face and neck. She was hospitalized.

On September 4, 2025, an employee was performing crane training for a group of employees, which included engaging in practical testing. At the end of the day, he began experiencing muscle stiffness as he was getting ready to leave. On the way home, the employee experienced blurred vision and nausea, resulting in hospitalization due to a kidney injury caused by dehydration.

An employee was descending a 10-foot fixed ladder. The employee fell from the ladder to the concrete floor 8 feet below and suffered a broken hip.

An employee was checking for air flow from an automobile's engine fan. The fan caught his left hand and lacerated his index, middle, ring, and little fingers, with a partial amputation of the little finger.

An employee was conducting maintenance on an obstacle course. He was securing the bottom section of a two-part extension ladder to a tree by screwing bolts into the tree. He began ascending the ladder again with a blue strap to secure the ladder to the tree when the ladder began sliding on the tree and he fell approximately 8 feet to the ground. The employee sustained fractures to a rib on the left side, his sacrum, and three parts of his pelvis, as well as abrasions.

An employee was cleaning when a door closed on her right hand, resulting in the partial amputation of a fingertip.

On 7/22/2025, an employee was moving some room dividers when their left index finger was caught in one of the dividers. The employee's fingertip was amputated without loss of bone.

An employee was working on rooftop HVAC equipment and bumped their elbow on the equipment resulting in a puncture wound. The employee was hospitalized for cellulitis and septic bursitis.

An employee was standing in the parking lot while waiting for another employee to move a cart with a sign when they were struck by a vehicle, resulting in four fractured ribs.

An employee was driving a utility task vehicle (UTV) around campus collecting trash. While performing a U-turn the UTV became lodged against the curb. While freeing the UTV, it launched forward and collided with a wall adjacent to the curb. The steering wheel jerked and fractured their wrist. The employee was hospitalized and required surgery.

An employee was walking down the stairs from the second floor to the first floor when she lost her balance and fell down the stairs. The employee sustained broken ribs.

An employee passed through metal swinging doors and stopped outside the doors to have a conversation with another employee. The doors swung back and hit the injured employee in the back, resulting in hospitalization due to muscle and nerve pain in the spine.

An employee stepped out of her office, turned to lock the door, turned back, slipped, and fell. She suffered a broken right femur and was hospitalized. There were water droplets on the floor from a wet umbrella.

A student employee was moving a shipment of steel bar stock (approximately 3/4 by 3 by 10-12 ) in the stock room by himself. Shortly thereafter, another employee and two students heard a loud noise. They discovered the employee unconscious underneath a pile of bar stock weighing approximately 916 pounds. The student employee sustained skull fractures, brain swelling, a collapsed lung, and a spinal cord injury.

An employee was driving a vehicle through a tunnel on a campus road to the dumpsters. The bed was elevated and contacted the tunnel structure. The employee sustained a lacerated pancreas and a dislocated shoulder.

An employee was on a ladder that was leaning against a wall inside a multipurpose room. He was running wiring through the walls to install sound equipment. When he reached up to pull a wire, the ladder slid down the wall to the floor. The employee fell on top of the ladder and struck the floor, fracturing multiple bones in his face and his left hip.

An employee was leaving the building when they tripped on the steps and fell to the ground. The employee sustained a fractured left foot and a sprained right ankle.
