The motor on the feed bin was locked out at the time.

When she went to the operator to point out where it was jammed, her glove was pulled into the cutter head and her index and middle fingertips were amputated.

While an employee was moving lumber in row one using a diesel-powered forklift, the top two packs of lumber in row two fell onto the forklift, causing the forklift cage to fail. The cage then struck the employee, fracturing his scapula and dislocating his collarbone. He was hospitalized.
