A T-shaped stainless steel vacuum header was being rigged to forks using straps. After the pipe was lifted, an employee was moving to rotate the header when the strap slipped off the fork. The pipe fell onto the employee's right big toe and second toe, causing multiple fractures.

At about 10:45 a.m. on March 7, 2019, an employee was at the back of a printing press, where dried paper had wrapped around a carrying roll. He was 3 feet up a 6-foot ladder, removing the stuck paper, when he fell to the cement floor, encountering wall-mounted valves, a metal catch tray on the floor, and a metal grating. He suffered back pain and two lacerations, one to the back of the head and one on the left elbow. Each laceration required stitches.
