An employee was stringing roving through the machine using polyester cord strapping. He was using his left hand to keep tension on the strapping as it fed through the tensioner. The strapping wrapped around his right thumb and pulled his hand into the tensioner. The strapping removed skin and flesh to the bone from just above the knuckle to the thumb tip. The employee was hospitalized.

An employee was standing on the step on the passenger's rear side of a bucket truck returning the empty basket and boom to its cradle position. As the boom lowered the employee was caught between the control station and the empty bucket. They sustained an upper extremity fracture, an injury to lower neck vertebrae, and a blood clot.

An employee was cooking a batch of lubricating grease. Flames and grease slurry came out of the grease cooking kettle and splashed onto the employee; this also started a fire outside the kettle. The grease/oil slurry was approximately 185-200 degrees and caused second degree burns to the employee's face, neck, arms and hands.
