An employee was wearing cut-resistant gloves while operating an automatic slitter. The machine stalled and the employee placed their right hand on a panel to clean it before cutting. The slitter started running again, catching and damaging the cut-resistant glove. The employee's hand made contact with the slitting blades, resulting in amputation of the right ring finger and injury to two other fingers and lacerations to the hand. The employee was hospitalized. The machine was not guarded at the time.

A line leader was crouched down observing the chain and sprocket on a carpet filling machine for maintenance or repair when another employee got his right index finger caught between the chain and sprocket. Skin and the fingernail were removed and the employee required surgical amputation of the fingertip to the first knuckle.

Two employees were adding wire pull boxes to two empty conduits. Employee 1 loosened a coupling on a conduit and opened it to check if it was empty. Wires were visible, so the employee went to close the conduit again. Wires in the conduit became pinched and a wire's insulated jacket was damaged. Electricity in the wire (277 volts), grounded out through employee 1 and he was unable to let go of the conduit. Employee 2 pulled employee 1 away and was also shocked. Both employees were hospitalized for electrical shock.
