Employee injured when elevated work cage falls
Investigation abstract
Employee #1 was in a duct shaft installing duct from a work cage that was raised and lowered by an electric hoist. He incorrectly hooked the hoist to the work c age. The hook came loose, allowing him to fall. He sustained multiple bruises an d contusions to his arms, but did not require hospitalization.
