Investigation abstract

EMPLOYEE #1 REACHED INTO THE MACHINE'S POINT OF OPERATION TO GET A PART AS THE R AM WAS ON ITS UPWARD STROKE. HIS FINGERS WERE CAUGHT AND AMPUTATED BETWEEN THE B OTTOM OF THE RAM AND THE METAL STOP BAR. THE STOP BAR WAS ONE INCH THICK, 2 INCH ES WIDE AND 7 INCHES LONG.
