EMPLOYEE'S FINGER AMPUTATED BY TABLE SAW BLADE
Investigation abstract
EMPLOYEE #1 WAS USING A TABLE SAW WITH AN UNGUARDED BLADE TO CUT PIECES OF PLAST IC. THE TOP EDGE OF THE BLADE WAS ABOUT 2 INCHES ABOVE THE MATERIAL BEING CUT. T HE EMPLOYEE'S LEFT HAND SLIPPED, ALLOWING THE INDEX FINGER OF THAT HAND TO COME IN CONTACT WITH THE SAW BLADE. THE FINGER WAS AMPUTATED, BUT LATER REATTACHED AT THE HOSPITAL. PUSH BLOCKS HAD BEEN PROVIDED FOR THE EMPLOYEE'S USE.
