Employee's fingertip amputated in machine

Investigation abstract

Employee #1 was removing plastic buildup from a flash chute when he accidentally stuck his finger into the 1 1/2 in. diameter by 1 in. deep grease fitting hole. His fingertip was amputated by the vertical movement of the machine.
