Employee injured in fall when floor collapses
Investigation abstract
Employee #1, a construction worker with approximately one year of experience wit h the employer, was removing 4 ft by 8 ft by 1/4 in. sheets of plywood from a bu ilding. He was throwing the plywood off the side of the structure when the concr ete floor system beneath him collapsed. Employee #1 fell with the flooring, sust aining a dislocated neck and a sprained wrist. He later stated that the wind was strong enough to whip around the plywood as he was working.
