ONE EMPLOYEE INJURED DURING FALL WHEN FLOOR COLLAPSES

Investigation abstract

On November 1, 1985, a floor system collapsed, carrying Employee #1 through the floors below. Employee #1 sustained a dislocated neck and a sprained wrist when the collapsed floor came to rest on a concrete floor two floors below. The emplo yee was sorting 4-ft by 8-ft sheets of 0.25-in plywood by pushing them to the fl oor below when the floor system Employee #1 was working on collapsed. Employee # 1 stated the plywood work was performed on windy day.
