EMPLOYEE'S HAND CAUGHT IN MACHINE
Investigation abstract
Employee #1, a mechanic, was taking measurements to make adjustments to correct a problem with a Meyers labeler. The labeler was applying labels to plastic bott les of liquid soap. The labels were not getting enough glue and were falling off of the bottles. Employee #1 reached 20 in. over the guard on the machine while it was running to take a measurement near the moving wiper pad cylinder assembly . The guard was 59 in. from the floor. His foot slipped on something on the floo r and his hand got caught and was crushed between the station frame and the wipe r section movement.
