Employee killed when crushed between truck and storage
Investigation abstract
Four employees were standing in an area at the rear of a 40-ft-long storage trai ployee #1 was killed when the truck crushed his chest. The employees had not rec eived training on the recognition and avoidance of hazards associated with stayi ng clear of trucks backing up. The employees had not received training in safely directing the truck driver. ler. The employees were waiting for a 1983 International, two-axle van-type truc k with a 24-ft-long body. The truck was backing up to the storage trailer so tha t it could be loaded with cartons of Class C fireworks. An employee was standing on the driver's side of the truck directing the truck driver. Another employee was standing on the passenger side of the truck. Employee #1 and a coworker were standing directly behind the truck. The truck was moving backwards, on an angle , and traveled further than intended.
