Investigation abstract

EMPLOYEE CLEANING ROLLER BENEATH CONVEYOR BELT WITHOUT FIRST LOCKING OUT EQUIPME NT. HAD HIS ARM CAUGHT BETWEEN THE ROLLER AND THE BELT. EMPLOYEES ARM WAS BROKEN . EMPLOYEE WAS TRAINED TO LOCK OUT ALL MACHINERY BEFORE WORKING ON SAME.
