EMPLOYEE STRUCK AND KILLED BY END OF WHIPPING STEEL HOSE

Investigation abstract

EMPLOYEES WERE TRYING TO RUN SURFACE CASING INTO A WELL, BUT BORE CUTTINGS, WHIC HIS ACCIDENT COULD HAVE BEEN PREVENTED BY SECURING THE STANDPIPE END THE HOSE WI TH A SAFETY CHAIN. H HAD SETTLED TO THE BOTTOM, WERE GETTING IN THE WAY. TO ALLEVIATE THIS PROBLEM, MUD WAS CIRCULATED THROUGH THE CASING TO FLUSH OUT THE CUTTINGS. THE MUD WAS CI RCULATED BY CONNECTING A STEEL ROTARY HOSE FROM THE STANDPIPE TO A SWEDGE IN THE TOP OF THE CASING STRING. WHEN THE NEXT-TO-LAST JOINT OF CASING WAS BEING WORKE D INTO THE HOLE, A NIPPLE WHICH CONNECTED THE STEEL HOSE TO THE STANDPIPE EXPERI ENCED THREAD FAILURE AND PULLED OUT OF THE COLLAR TO WHICH IT WAS ATTACHED. THE HIGH INTERNAL PRESSURE ON THE MUD CAUSED THE THE END OF THE HOSE TO WHIP AROUND ITS ANCHOR AT THE SWEDGE. THE FLYING HOSE-END STRUCK EMPLOYEE #1, KILLING HIM. T
