EMPLOYEES DIED WHEN ENTERING AN OXYGEN DEFICIENT ENVIRONMENT
Investigation abstract
Two employees died from asphyxiation when they entered an oxygen deficient tank truck that was cleaned with nitrogen. The employees did not wear any respiratory protection or follow confined space procedures.
