EMPLOYEE STRUCK AND KILLED BY A BAR CAUGHT IN A PRESS
Investigation abstract
On April 27, 1985, an employee was using a steel pry bar to open the air collar on the pulp press while the press was operational. The electric motor of the pre ss was operating at speed of 45 rpm and power of 50 HP electric motor while the employee was working on the unit when one of the brackets joining the collar to shaft broke free and struck the 27 lb, 64 in. by 1.25 in. pry bar. The bar dropt into the running motor and spun towards the individual striking him on the left side of head and jaw causing fatal traumatic fracture (traumatic brain injury) and resulting in a fatality.
