ONE KILLED, ONE HOSPITALIZED WHEN PLATFORM HOOK FELL
Investigation abstract
Employee #1 and Employee #2 were working from a work platform suspended from a c rawler crane. The wire rope came loose from the block and hook holding the platf orm. The platform fell about 30 feet. The block and hook assembly, which weighed over 300 lb, fell on the employees crushing them. Employee #1 was killed and Em ployee #2 was hospitalized with broken bones.
