EMPLOYEE #1 WAS LOADING RUBBER INTO AN OVERHEAD CONVEYOR. SOMEHOW HIS HANDS BECA ME CAUGHT IN THE CONVEYOR. HE WAS PULLED IN, ON HIS BACK, AND CRUSHED TO DEATH.
