EMPLOYEE #1 HAD COMPLETED LAYING A 30 INCH BY 8 INCH STORM DRAIN TILE WHEN THE S OUTH WALL OF THE 9 FOOT DEEP TRENCH CAVED IN. THE EMPLOYEE WAS PINNED AGAINST TH E PIPE AND CRUSHED TO DEATH.
