EMPLOYEE PINNED UNDER CAR DIES FROM ASPHYXIATION
Investigation abstract
Employee #1 was performing unauthorized work on a car after hours removing a tra nsmission. The car was located in the car yard on stands and became unstable and tilted off of the stands onto Employee #1. Employee #1 was found dead under the car from asphyxiation. No violation of a standard was cited due to employee mis conduct.
