FALL FROM STEEL SUPPORT BEAM
Investigation abstract
EMPLOYEE #1 WAS WORKING AS A RIGGER, PREPARING FOR THE "KISS" CONCERT. SOMEONE S MELLED SMOKE SO EMPLOYEE #1 TOOK A PORTABLE FIRE EXTINGUISHER UP TO THE CEILING AREA TO CHECK FOR FIRE. HE WAS WALKING ON A STEEL BEAM 47 FEET, 9 INCHES ABOVE T HE FLOOR. NO FALL PROTECTION OF ANY KIND WAS IN USE. HE FELL TO THE CONCRETE FLO OR BELOW AND WAS KILLED.
