Event description

CONSTRUCTION EMPLOYEE KILLED WHEN SCAFFOLD OVERTURNED

Investigation abstract

AT APPROXIMATELY 10:40 AM ON JANUARY 5, 1987, EMPLOYEE #1 WAS SEWING SECTIONS OF HALF INCH NYLON MESH NETTING TOGETHER. THE NETTING HAD BEEN PLACED UNDER A CANO PY SEVERAL WEEKS AGO. HE WAS WORKING FROM A 19 INCH WIDE BY 26 INCHES LONG WORK PLATFORM, 11 1/2 FEET ABOVE GROUND LEVEL. THE "HI-JACKER" ECONOMY LADDER HAD BEE N PLACED ON AN 18 INCH WIDE AREA OF SIDEWALK, NEXT TO A 6 INCH HIGH CURB. THERE ALSO WAS AN INCLINE FOR A HANDICAPPED RAMP. IT IS PROBABLE THAT THE SCAFFOLD MOV ED, DROPPED OFF THE 6 INCH CURB AND FELL OVER. EMPLOYEE #1 DIED OF A BROKEN NECK , FACIAL FRACTURES AND CUTS AND BROKEN RIBS.
