CLEANING A FIBER FROM A BLADE CAUSES A FINGER AMPUTATION

Investigation abstract

Employee #1 was cleaning a fiber fragment under the slitter blade in a level two compactor machine. He caught his left index finger under the blade, resulting i n an amputation at the first joint.
