EMPLOYEE STRUCK ON HEAD BY FALLING BALE OF WOOD PULP

Investigation abstract

ON NOVEMBER 23, 1986, EMPLOYEE #1 WAS OPERATING A FORKLIFT TRUCK. HE HAD 5 BALES OF WOOD PULP ON THE TRUCK, AND WAS ATTEMPTING TO PUSH THEM FURTHER UP ON THE FO RKLIFT. HE PUSHED THE LOAD AGAINST THE BOTTOM OF ROWS 4 AND 5, CAUSING 8 BALES T O FALL FROM THE TOP OF ROW 4. EMPLOYEE #1 WAS LEANING OUT THE LEFT SIDE OF THE T RUCK AND WAS STRUCK ON THE RIGHT SIDE OF HIS HEAD BY ONE OR MORE OF THE FALLING BALES. HE DIED LATER THE SAME DAY FROM A SKULL FRACTURE AND CEREBRAL TRAUMA.
