Event description
Employee injures head in 12 ft fall from elevated platform
Investigation abstract
Employee #1 was on the platform of a scissor hoist applying insulating material to the first floor ceiling of the narthex of a grandstand. The work platform was elevated approximately 12 ft and the safety chain and bar that closed off acces s to the hoist platform was not secured in place. While applying the insulating material, Employee #1 walked backward off the elevated platform and fell to the floor. He landed on his feet, dropped to his buttocks, and fell backward, striki ng his head against the concrete floor. He was hospitalized. He had removed his hard hat and was wearing a baseball cap at the time of the accident.
