EMPLOYEE #1 WAS STRUCK BY A 4-FOOT PIPE (DIPSTICK) WHEN TRYING TO REMOVE THE CAP FROM A PRESSURE VESSEL. HE FAILED TO LOWER THE HOIST AND BLEED THE LINE BEFORE OPENING THE VESSEL.
