EMPLOYEE #1 WAS A GRADEMAN. HE WAS MEASURING GRADE WHEN A CATERPILLAR MODEL 14G GRADER BACKED OVER HIM, CRUSHING HIS HEAD AND KILLING HIM INSTANTLY. THE BACK-UP ALARM WAS AUDIBLE AND OPERATIVE.
