EMPLOYEE CRUSHED TO DEATH UNDER LOADER TRANSMISSION

Investigation abstract

EMPLOYEE #1 WAS INSTALLING A TRANSMISSION IN A FRONT END-LOADER. HE WAS USING A HYDRAULIC BOOM WINCH TO RAISE THE TRANSMISSION. A PORTABLE SLAVE UNIT WAS BEING USED TO CONTROL THE WINCH. FOR EASE OF ACCESS BY EMPLOYEE #1, THE CONTROL PANEL WAS ORIENTED TO HIM UPSIDE DOWN. HE ATTEMPTED TO ACTIVATE THE WINCH, BUT, BECAUS E OF THE UNFAMILIAR ORIENTATION OF THE CONTROL PANEL, ACTIVATED THE BOOM INSTEAD . THE TRANSMISSION LOWERED ONTO HIS CHEST, CRUSHING HIM.
