Event description
Two employees injured in fall from collapsing scaffold
Investigation abstract
Employees #1 and #2 were told by their foreman to pick up some forming component ench accidentally hit the brace, displacing it enough that the scaffold could no t support the additional weight of the employees. s, load them into a wheelbarrow, and bring them downstairs. The employees took a shortcut past a perimeter barricade and onto a form scaffold, on which was writ ten "do not stand." They then planned to throw the components over the edge of t he scaffold to the ground. When both employees stepped onto the scaffold it coll apsed, causing them to fall to the ground. Employees #1 and #2 were hospitalized . They stated that they did not know why the scaffold collapsed.
