EMPLOYEE #1 RECEIVED SEVERE HEAD INJURIES WHEN THE BLANK HOLDER (UPPER DIE) FELL ON HIM DURING SET-UP ADJUSTMENTS ON A POWER PRESS. HE DID NOT USE SAFETY BLOCKS . THE EMPLOYEE DIED FROM HIS INJURIES. (THE PRESS DID NOT CYCLE.)
