UTILITY POLES FELL FROM TRAILER BED AND STRUCK EMPLOYEE
Investigation abstract
An employee was unloading utility poles from a flatbed tractor-trailer. The load broke loose, causing several utility poles to fall from the trailer. The poles struck and killed the employee.
