Ignition in Drilled Hole Caused Blow-Back; Employee Killed
Investigation abstract
A drilling crew encountered difficulty at the 30-foot level when drilling into a n unknown combustible material. While the crew was extricating the drill bit the material in the hole ignited. The ensuing explosion, which erupted up the drill ed hole, killed employee #1.
