EMPLOYEE #1 WAS STRUCK ON THE HEAD BY A 2 BY 6 INCH BOARD AFTER HE HAD REMOVED T HE JACK THAT SUPPORTED THE BOARD. THE BOARD WAS A SUPPORT MEMBER FOR FLYING FORM S. EMPLOYEE #1 WAS WEARING A HARD HAT AND THUS AVOIDED A DISABLING INJURY.
