Event description

EMPLOYEE KILLED WHEN STRUCK IN ABDOMEN BY FLYING STEEL ROD

Investigation abstract

EMPLOYEE #1 HAD "CHUCKED-UP" A HALF INCH DIAMETER, 16 INCH LONG STEEL ROD IN A M ILLING MACHINE/DRILL PRESS. HE SPEEDED UP THE MACHINE TO 3400 RPM TO POLISH THE ROD WITH EMERY CLOTH. CENTRIFUGAL FORCE ON THE ROD CAUSED IT TO BREAK AND FLY OU T OF THE MACHINE. A PART OF THE ROD STRUCK EMPLOYEE #1 IN THE ABDOMEN, LACERATIN G HIS AORTA. THE EMPLOYEE DIED OF THE INJURY. THE COMPANY DID NOT HAVE ANY WRITT EN PROCEDURES OR SAFETY RULES FOR THE OPERATION OF THIS MACHINE.
