FELL FROM LADDER; STRUCK HEAD ON FLOOR
Investigation abstract
EMPLOYEE #1 WAS ASIGNED TO BUFF THE ELECTRODE HEADS OF A MELTING FURNACE IN A ST EEL MILL. ACCESS TO THE TOP OF THE FURNACE IS BY FIXED METAL LADDER. THE LADDER EXTENDS FROM A METAL PLATFORM 50 INCHES FROM THE CONCRETE FLOOR AT THE BASE OF T HE FURNACE. AS EMPLOYEE #1 WAS CLIMBING UP HE LOST CONTROL AND FELL, BACKWARDS, FROM THE LADDER. HE FIRST STRUCK THE TOP RAILING OF THE METAL PLATFORM AND THEN LANDED, HEAD FIRST, ON THE CONCRETE FLOOR. HE SUFFERED FATAL HEAD INJURIES.
