CONSTRUCTION EMPLOYEE KILLED WHEN STRUCK IN HEAD BY PIPE
Investigation abstract
EMPLOYEE #1 (A SUPERINTENDENT) WAS STANDING BESIDE A TRACK PROPELLED BACKHOE, DI RECTING A PIPE-LAYING OPERATION. THE BACKHOE WAS BEING USED TO LIFT 55 FOOT LENG THS OF 15 INCH DIAMETER STEEL CASING, SWING THE CASING OVER THE TRENCH AND LOWER IT INTO PLACE. AS A LENGTH OF CASING WAS BEING MOVED TOWARD THE TRENCH, ONE END CAUGHT IN THE BANK OF THE DITCH. EMPLOYEE #1 FREED THE STUCK END BY PULLING ON A ROPE TIED TO IT. WHEN THE CASING WAS FREED, ONE END SWUNG ACROSS THE DITCH AND STRUCK EMPLOYEE #1 IN THE HEAD. HE WAS KILLED WHEN HIS HEAD WAS CRUSHED BETWEEN THE PIPE AND THE BACKHOE CAB.
