EMPLOYEE #1 USED A BOTTLE TYPE PORTABLE JACK TO RAISE A BUS SO THAT HE COULD CRA WL UNDER AND MAKE REPAIRS. THE RAISED BUS WAS NOT CRIBBED, BLOCKED, OR OTHERWISE SECURED. THE JACK FAILED, ALLOWING THE BUS TO FALL AND CRUSH EMPLOYEE #1 TO DEA TH.
