EMPLOYEE CRUSHED TO DEATH UNDER HEAVY MACHINERY

Investigation abstract

AT APPROXIMATELY 9 AM ON APRIL 29, 1987, EMPLOYEE #1 AND ANOTHER EMPLOYEE WERE A TTEMPTING TO STEADY A LAGUN MILLING MACHINE, LOADED ON THE FORKS OF A CLARK FORK LIFT, BY HAND. THE MACHINE WAS BEING LOADED ONTO A FLATBED TRUCK. WHEN THE MACH INE BEGAN TO TILT TOWARDS EMPLOYEE #1 HE ATTEMPTED TO STOP IT, BUT SOON REALIZED IT WAS TOO HEAVY FOR HIM TO HANDLE. HE TRIED TO BACK AWAY. MEANWHILE THE MACHIN E FELL OFF THE FORKLIFT FORKS AND ONTO EMPLOYEE #1. HE SUFFERED BROKEN LEGS, A B ROKEN PELVIS AND A CRUSHED ABDOMINAL CAVITY. HE DIED 3 DAYS LATER.
